The referral will be acknowledged within 3 days.

Post Acute Care

Schoolcare Program

The referral has three sections. One must be completed by the school, one by the family and one by the child/adolescent's treating physician. Link to Schoolcare Program referral. The referral is valid for a 12 month period

NB: Children who attend Catholic schools must complete a separate referral form, which can be accessed from the Catholic Education Melbourne.

Homecare Program

Referrals can be received from either within the hospital setting or from the community. Referrals must be supported by a confirmed funding source prior to consideration for acceptance.
