    Children being discharged from the RCH are eligible for PAC support if there is an increase in medical care need for the patient as a result of the hospitalisation and parent/guardian require support to successfully transition home.  

    Post-acute care services provide individually tailored packages of services to patients who have one or a combination of:

    • A short-term need for community nursing intervention.
    • A short-term need for single discipline allied health intervention.
    • A short-term need for community-based supports on the account of the child’s healthcare, psychological needs and home environment.

    The need for services must be linked to their hospital presentation and:

    • Parent/guardian (or patient if age appropriate) needs to consent to the services.
    • Patient must be a Victorian resident and live within 50km radius of RCH or have a regional healthcare service able to provide relevant care.
    • Patient must have a valid medicare number.
    • PAC is unable to provide support to families who are choosing not to follow the recommendations of their RCH medical team.
    • Any education required by the parent regarding new medical care needs is commenced on the ward.
    • Safe care environment at home, no threatening patient or guardian behaviours/code greys during admission or any OHS concerns.
