The Post Acute Care (PAC) program provides short-term (up to four weeks) supports at home to children whose healthcare needs have increased following an acute hospital admission or Emergency Department presentation. The program aims to help children recover at home and prevent readmission to hospital.
The PAC services is a Victorian statewide initiative available to a patient as long as the admission was to The Royal Children's Hospital (RCH). The services may be provided by the RCH staff members or external community organisations. External community-based organisations will be used for patients and families who live outside of the RCH catchment area.
We offer Nursing, Physiotherapy, Occupational Therapy and In-home supports within the service.
The Program’s ultimate goal is to improve and maintain a Patient/carers independence within the community, supporting a smooth transition from inpatient setting to home environment.
