Referrals

  • It is the responsibility of the referrer to have a conversation with the family to discuss their potential inclusion within the service.

    How to make a referral via EPIC: 

    We encourage you to call the program coordinator to discuss your referral and determine eligibility and availability of the service for your patient.

    Limitations of the service to consider

    • Post Acute Care operates business hours Monday to Friday only.
    • There is no after hours clinical supports provided to patient’s on the program.
    • Patient’s who require equipment or consumables as part of their care or intervention is not funded by the PAC service.
    • Patient’s can receive Post Acute Care services for up to four weeks, parents/carer must be willing to undertake any ongoing cares/ medical intervention required by the patient.
