The attached form must be completed inconjuction with a request form and they must accompany the sample to the laboratory (SR-E-022).

Samples must be collected prior to that day's treatment (for trough level assessment).

Samples will be transported weekly to testing laboratory (Wednesdays).

COLLECTION NOTES:
Place EDTA on an ice pillow and send to Specimen Reception immediately.

LABORATORY NOTES:
Centrifuge sample for 15 minutes at 4oC at 3500 rpm.
Aliquot plasma and store in the -70oC freezer.
Transport on dry ice with next routine courier.
