Joint RCH and adult service transition clinics

The Royal Children's Hospital (RCH) works collaboratively with many adult health services across Victoria and offers the option of joint RCH and adult service transition clinics. The RCH Transition Support Service currently runs more than 70 joint transition clinics annually to support young people and families in meeting with their new adult health care teams before being discharged from the RCH.

Here are some of the medical and surgical units who run joint clinics for young people and their families, in partnership with adult health services.

• Cardiology – complex congenital heart disease, heart transplant, pulmonary hypertension
• Gastroenterology and hepatology – inflammatory bowel disease, liver transplant
• General Medicine
• Haematology – sickle cell
• Immunology
• Metabolic Medicine
• Nephrology
• Neurodevelopment and disability – cerebral palsy, spina bifida, other
• Neurology – epilepsy, neuromuscular, neurofibromatosis
• Oncology
• Paediatric surgery – oesophageal atresia and trachea-oesophageal fistula, hirschsprung’s disease and anorectal malformation
• Rehabilitation – acquired brain injury, spinal cord injury
• Respiratory – cystic fibrosis, sleep, other respiratory concerns
• Urology
